For phone clients, it is easiest to open the .rtf Intake Form, fill it out and email it back to me (or discuss with me if you prefer to snail mail or fax the .pdf Intake Form). In-office clients have the option of bringing or arriving early to fill out paperwork. Keep a copy of the Informed Consent and Wellness/Services Agreement forms for your records. Once you have fully reviewed these, agree to the terms, and are ready to send me the filled in Intake, please put into the Subject, Wellness Intake _(with your initials here), then click on the new client return email link to send – and attach your Intake that has been filled out and re-named with your initials added EF_Intake _(your initials here).
